Ask a room of maintenance people which machine is the plant's worst actor and you'll get four answers. This toolkit settles it with arithmetic.

Every plant has the same hidden pattern: a small share of assets causes most of the failure cost, downtime, and emergency work — but the events are recorded one at a time, closed one at a time, and forgotten one at a time, so nobody ever sees the pile. The Bad Actor & Jack-Knife Analysis Toolkit stacks your failure history up, ranks it, and — the part most bad-actor lists skip — tells you what kind of problem each asset actually is.

WHAT IT DOES
• Turns your failure log into a per-asset rollup: failure count, total downtime, total cost, average cost per failure, share of total cost, and a Pareto rank by total cost
• Classifies every asset with the classic jack-knife logic — failure FREQUENCY vs. average CONSEQUENCE per failure — using thresholds drawn at the medians of your own data (with a documented manual override)
• Assigns one of four quadrants, each with a different playbook: CRITICAL (high/high), CHRONIC (fails often, cheap each time), ACUTE (fails rarely, costs a fortune), MONITOR (background noise)
• Dashboards the plant: total events, downtime and cost, a Top 10 bad actors table with cumulative share, Pareto concentration (what the Top 5 / Top 10 hold), quadrant counts and cost shares, plus reconciliation checks that flag log rows whose asset name matches nothing on the rollup
• Plots the jack-knife scatter (log-scale cost axis) and a quadrant map table
• Auto-generates an Action Plan: every classified asset gets its phase and the playbook's first action, with blank owner / due / status columns for your review meeting
• Ships EMPTY — and includes a fully worked fictional plant (Blue Ridge Foods: 14 assets, 60 events, $131,360 of failure cost, where the top 3 assets hold $71,000 of it) on its own tab, so you can watch the engine think before you trust it with your plant

HOW IT WORKS — 5 STEPS
1. SET THE WINDOW — one analysis period for every asset (twelve trailing months is the standard)
2. LOG THE EVENTS — one row per failure: asset, date, failure mode, downtime, and cost in four buckets (labor, parts, lost production, other); the total calculates itself
3. READ THE ROLLUP — counts, costs, averages, shares, and Pareto ranks calculate from the log with standard formulas
4. READ THE QUADRANTS — median thresholds classify every asset; the Dashboard shows where the money sits and which kind of problem each asset is
5. WORK THE PLAN — CRITICAL first (contain, then root-cause), then CHRONIC (kill the dominant failure mode) and ACUTE (cut the consequence); MONITOR assets get nothing, on purpose

Designed by a Reliability Engineer.

This product was designed by a Reliability Engineer with 15+ years in the industry, including experience across defense, aerospace, and weapons programs. He has built and implemented reliability-centered maintenance programs at multiple facilities — so the logic in these pages is not textbook theory. It is the same structure he has put into practice on real equipment, with real maintenance teams: the calculations are shown, not hidden — every number in this workbook can be traced back to its inputs and its method. Every worksheet is built to be used the way the work is actually done — in a session, on a plant floor, with the people who own the equipment.

Free updates included — if this product is corrected or improved, the updated edition is delivered free to everyone who purchased it, via the store.

WHAT'S INCLUDED
• Bad-Actor-JackKnife-Toolkit.xlsx — 6 tabs: Start Here, Failure Log (200 event rows), Asset Rollup & Jack-Knife (60 assets), Dashboard with jack-knife scatter chart, auto-generated Action Plan, and the fully worked example. Standard formulas only — no macros — works in Microsoft Excel AND Google Sheets
• Bad-Actor-JackKnife-Guide.pdf — 18-page guide: the jack-knife method in five steps, what counts as a failure event and how to capture cost without lying, medians/thresholds/overrides explained, a playbook chapter for each quadrant, the complete worked example including its full 60-event log, how to run the bad-actor review meeting, the limits of the method stated plainly, and two printable worksheets

WHO IT'S FOR
Maintenance managers, reliability engineers, and planner/schedulers at any plant with failure history — including plants with no reliability department. Also used by consultants running bad-actor reviews for clients.

WHAT IT'S NOT / SCOPE
This is an analysis and prioritization tool — it is not a CMMS and it does not connect to your systems. A quadrant is a prioritization, not a verdict on a machine, a crew, or a vendor. Safety-critical and protective functions follow your site's risk processes on their own track, regardless of where any cost plot puts them. Classifications are derived entirely from data you enter; the worked example is fictional and its figures are for instruction.
